Consent for Participation: I, the undersigned, hereby give my consent for my child, named above, to work with the St. John's Minor Baseball Association. I understand that the work may involve various tasks related to the association's operations, including but not limited to, assisting with game-day logistics, general maintenance, team organisation, or other duties as determined by the association. Parent/Guardian Contact for Safety and Scheduling: I understand that for safety and organisational reasons, the St. John's Minor Baseball Association will contact me directly regarding my childs scheduled work shifts, as well as any absences or adjustments to their scheduled shifts. I agree to respond to these communications in a timely manner to ensure my childs participation and safety during their determined schedule. Respond with: Accept or Decline

Liability and Safety: I acknowledge that my child will be participating in work activities under the guidance of St. John's Minor Baseball Association staff and volunteers. Work activities may be supervised or unsupervised. I trust that the organisation will provide a safe environment, and I agree to notify the organisation of any concerns regarding my childs safety or well-being. Respond with: Accept or Decline

Parental Acknowledgement: I, the parent/guardian of the above-named child, acknowledge by my name below that I have read, understood, and agree to the terms of this consent form. I authorise my child to participate in work activities for the St. John's Minor Baseball Association and commit to staying in communication regarding their work schedule and attendance.
